Background Nocturnal enuresis or bedwetting is the most common type of urinary incontinence in children. It has significant psychological effects on both the child and the family. The Aim of study was to evaluate the role of UDS in management of refractory Nocturnal Enuresis (MNE). Patients and MethodsThis Prospective Study was carried out in Urology Department, Zagazig University Hospital that included 24 Patients, 14 boys and 10 girls. The patient ages was ranged from 10–18 years old with mean age (SD) of 11.91± 1.95 years. All patients were treated with mono pharmacotherapy (desmopressin). The duration of treatment before UDS at last six months. Results bladder sensation was intact in 41.7%, increased in 50.0% and decreased in 8.3% of patients. The value of a single test PVR was ranged from 20-150 ml. 21 of patients were no post voiding volume (87.5%) and 3 cases had post voiding volume (12.5 %). The filling cystometry was normal in 6 patients (25%) and abnormal in 18 patients (75%). Bladder sensation intact in 10 patients (41.7%) and decrease in 2 patients (8.3%) and increase in 12 patients (50%) with p-value (< 0.05). conclusion: it is recommended to perform urodynamic studies in the form of uroflowmetry and cystometry for those with refractory nocturnal enuresis who failed ADHC ( desmopression ) for at last 6 months as initial monotherapy. Also a combination therapy of anticholinergics with desmopressin could be applied as treatment for those with refractory monosymptomatic nocturnal enuresis

Objectives: To compare the safety and efficacy of holmium laser enucleation of the prostate (HoLEP) and bipolar palsmakinetic enucleation of the prostate (BPEP) for patients with benign prostatic hyperplasia (BPH) and prostate volumes 50-120 mL.Methods: a total of 52 patients were prospectively randomized and enrolled in this study at Zagazig university hospitals. All patients were preoperatively evaluated using prostate volume measurement, the International Prostate Symptom Scores (IPSS) questionnaire, a quality-oflife (QoL) questionnaire and international index for erectile function-5 (IIEF-5). Measurement of maximum urinary flow rate (Qmax), post-void residual urine volume (PVR) and blood sample analysis for determination of prostate-specific antigen (PSA) and haemoglobin concentration. Patients were evaluated peri-operatively and postoperatively at 1, 3, 6 and 12 months with documentation of any complication. Results: Of 52 patients enrolled in this study, 47 patients completed 12 months of follow-up. There were statistically insignificant differences between both groups regarding the preoperative and intraoperative data. Perioperatively, bleeding, need for blood transfusion and hemoglobin drop were higher in the HoLEP group. There were no statistically significant differences in the post-operative follow-up data between both groups. Conclusion: In terms of surgical safety and efficacy BPEP is comparable to HoLEP for moderately to markedly enlarged prostates, with indication for surgical intervention.

Background Retrograde ejaculation is one of the most common complications in benign prostatic hyperplasia (BPH) surgery. Objectives the aim of this study was to evaluate the ejaculation and the urine flow of the patients after preservation of paracollicular gutter at the apex of prostate during transurethral resection of prostate. Patients & Methods This prospective clinical study was carried on 22 cases were recruited from the patients suffering from obstructing BPH who were admitted to Zagazig university hospital Department of urology, Faculty of Medicine, Zagazig University during the period from March 2018 to December 2018. Results This study shows that antegrade ejaculation is preserved in 19 patients (86.4%) while retrograde ejaculation seen in 3 patients (13.6%).no statistical differences were reported in International Index of Erectile Function (IIEF) at baseline and after 6 months. objectives: to compare between transurethral en-block ho-yag laser enucleation andtransurethral resection for papillary bladder cancer.methods: this is a prospective randomized study, carried out at urology department,zagazig university hospitals during the period from 15/7/2017 to 15/2/2018. aimed tocompare between laser en-block enucleation and monopolar tubrt for papillarybladder cancer as regard intraoperative outcomes (perforation , obturator jerk andbleeding ) and post operative outcomes ( wash time ,catheter time , hospital stay,blood transfusion ,residual )results: we studies 52 patients randomized in two groups, group for Ho-yag laser enblock enucleation and group for monopolar TURBT. We found that Ho-Yag laser wassuper to TURBT in term of intraoperative complications and postoperativeconclusions:Laser enucleation appear to be reliable procedure with many advantagesover monopolar TURBT , despite it need longer resection time ,it reduce intraoperativecomplications and post operative complications , it preserve tumor histologicalarchitecture and gives better tumor staging , it allow to remove the tumor completelyfrom it’s bed so decrease the probability of local persistence of malignant tissue andincidence of tumor recurrence .
