BackgroundPolysomnography (PSG) is the gold-standard diagnostic tool for Obstructive Sleep Apnea (OSA). However, the availability of PSG is limited, and OSA is widely underdiagnosed; more than 80% of most developed nations undiagnosed. ContextRespiratory support is an essential part in treating COVID-19 patients at risk for developing respiratory failure, and this become certain if associated with other comorbidities specifically obstructive sleep apnea (OSA).AimTo evaluate the role of early use of continuous positive airway pressure (CPAP) in management of moderate to severe COVID-19 patients at risk of OSA.Settings and designThis was experimental clinical trial.Patients and methodsEighty (80) patients with moderate to severe COVID-19 at risk of OSA were enrolled. They were simply randomized into two equal groups: non-CPAP group and CPAP group. Background: Soft tissue lesions are a common cause of pain and disability in elderly people, clinical examination alone has a limited value in deciding on the management options for the underlying etiology. Several studies have been done that evaluated the accuracy of either magnetic resonance imaging or high-resolution ultrasound in detection of shoulder disorders and only few studies compared these two methods. Aim of Study: To evaluate the role of high-resolution ultrasonography and magnetic resonance imaging in elderly patients with shoulder pain. Patients and Methods: In this cross-sectional study, a total of 22 patients (11 females and 11 males). Were presented with acute shoulder joint pain. The patients were aged from 60 to 70 years. Results: Overall, we were able to demonstrate performance values for musculoskeletal ultrasound in diagnosing the abnormalities of rotator cuff, biceps tendon, acromioclavicular joint, and glenohumeral joint compared to MRI to be 100% for sensitivity. Positive predictive value (PPV) was 100% for rotator cuff, biceps tendon, and glenohumeral joint disorders, while it was 96% for the abnormalities developed in the acromioclavicular joint. Finally, the accuracy was 98.4%, 100%, 96%, and 100% respectively for the abnormalities of rotator cuff, biceps tendon, acromioclavicular joint, and glenohumeral joint disorders, respectively. Conclusion: Ultrasound for the shoulder joint presents a high accuracy and sensitivity in diagnosis a wide spectrum of shoulder joint lesions, with a diagnostic performance value near to that of MRI. BackgroundObstructive sleep apnea (OSA) is a situation of repetitive and interrupted collapse of the upper airways (UAs) during sleep. The physiopathology of OSA is multifactorial. Provided that continuous positive airway pressure is the gold standard for treatment of OSA, the American Academy of Sleep Medicine in 2015 stated that mandibular advancement device (MAD) was indicated as a first-line therapy for mild OSA and a second-line therapy for moderate to severe OSA with lower BMI.AimThis study is aimed to evaluate the MAD success rate in patient with OSA and to assess improvement in sleep quality.

Background Coronaviruses are classified as pH dependent. Alkaline media induced by sodium bicarbonate (SB) could impede viral entry into cells. We aimed to study the possible role of SB as an adjuvant treatment of nonsevere COVID‑19 pneumonia. Methods The study included 182 adults with confirmed nonsevere COVID‑19 and chest computed tomography (CT) pneumonia; 127 assigned as study received conventional treatment plus adjuvant SB inhalation and nasal drops, as well as 55 assigned as control treated by conventional treatment only. Clinical and radiological assessments using chest CT score specific for COVID‑19 were done at days 0 and 30. Results Both the groups were comparable regarding demographic, clinical, and radiological characteristics. Clinical recovery was reported in 43/127 (33.9%) and 10/55 (18.2%) of the study and control groups, respectively (P= 0.03). The mean±standard deviation time to clinical improvement was 3.31±0.99 and 9.79±6.29 days for the study and control groups, respectively (P< 0.001). The median of the total chest CT score was reduced from 10 (4–15) to 3 (0–19) in the study group (P= 0.000) and from 13 (2–15) to 11 (2–19) in the control group (P= 0.53) on days 0 and 30, respectively. Conclusions SB could be a possible adjuvant therapy for selected patients with nonsevere COVID‑19 pneumonia.

تعبر مشکلة المديونية بشقيها الداخلية والخارجية عن المبالغ التى تلتزم بها الدولة نتيجة اقتراضها لتمويل العجز بالموازنة والتعهد بالسداد بعد مدة ودفع فائدة رصيد الدين حسب شروط إنشائه،وقد مثل الدين الداخلي والخارجي نحو76.44%،23.56%من الدين العام علي الترتيب خلال عام 2017،وتوجد علاقة تأثيرية تبادلية بين المديونية بشقيها وبعض المتغيرات الاقتصادية الکلية کالناتج المحلي الإجمالي والصادرات والواردات السلعية والخدمية......الخ وتتضح من خلال بعض المؤشرات يطلق عليها مؤشرات المديونية للحکم علي کفاءة إدارة المديونية في مصر.وتکمن مشکلة البحث أولا في تزايد المديونية وما ترتب عليه من ارتفاع سعر الفائدة والموجات التضخمية وانخفاض الاستثمارات نظرا لمحدودية المدخرات، وثانيا عند علاقة المديونية ببعض المتغيرات الاقتصادية الکلية کالناتج المحلي الإجمالي والصادرات السلعية والخدمية وإجمالي الاستثمار والادخار والاحتياطات الدولية الکلية حيث قدرت نسبة المديونية بتلک المتغيرات نحو102.50% ،512.64% ،601.56%، 608.67% ،790.49% للمتغيرات المدروسة علي الترتيب خلال الفترة (2000-2017) ،ونحو 99.19% ،418.65% ،538.33% ،495.12% ،529.96%علي الترتيب خلال الفترة (2000-2010)، ونحو107.95% ،704.76% ،716.26% ،841.96% ،1481.77%علي الترتيب خلال الفترة(2011-2017)، وثالثا في نمو عبء مدوفوعات الديون حيث قدرت نسبة خدمة الدين بالنسبة للمتغيرات الاقتصادية الکلية السابق الإشارة إليها بنحو2.02% ،10.09% ،11.84% ،11.98% ،15.56%علي الترتيب خلال الفترة الأولي ،ونحو2.19% ،9.24% ،11.88% ،10.93% ،11.70%علي الترتيب خلال الفترة الثانية، ونحو 1.77% ،11.58% ،11.77% ،13.84%،24.35%علي الترتيب خلال الفترة الثالثة،مما يستدعي معه ضرورة دراسة المديونية بشقيها الداخلية والخارجية والوقوف علي عبء مدفوعات الدين وعلاقتها بالمتغيرات الاقتصادية الکلية …
